Facility Use Requests

Would you like to host an event at the church? Fill out our Facility Use Request form!

Organizations or others desiring use of Church Facilities must submit a Facility Use Request as far in advance as possible to the date of the event.

Completed forms can be delivered or emailed to the Church – fpreshp@gmail.com or mailed to First Presbyterian Church of Holland Patent, PO Box 303, Holland Patent, NY 13354

You may complete the form electronically by downloading the form below and saving it to your computer. When you open your saved document, you may enter your information directly on the form by using the Fill & Sign Tools on the right-hand side of the screen. 

If you do not have access to a printer, blank forms are also available from the church secretary.
